Making a Referral

Patients can be referred to our clinic with or without a physician's prescription for the initial consultation.

If treatment is pursued, a prescription and progress notes to support the need for a cranial remolding helmet may be required for insurance processing.

We have expanded our in-network status and are now contracted with most major insurance carriers while still offering a deeply discounted Private-Pay option which can be a more efficient choice for some families.

We do accept patients with Medicaid, however, it is important to note that there is an additional step in the referral process. Forward Health requires that all prescriptions come from a cranial facial surgeon with a specialty in pediatrics. We are very fortunate to have Dr. Tina Sauerhammer seeing patients in Appleton and De Pere locations at the Wisconsin Institute of Plastic Surgery. Other specialist options include Milwaukee Childrens Hospital Craniofacial Dept, or UW Health- American Childrens Hospital Plastic Surgery Dept.
